What is transition care for seniors and other individuals?
Non-Medical, transitional care refers to the services an individual receives upon being discharged from hospital. It is expected that when a person, whether elderly or not, is coming home after a long or short-term stay at hospital, he or she will not have the energy to resume their everyday activities, and this includes their personal care and other activities that are vital to their health.

A lack of reliable transition services can, therefore, be counted among the major barriers to aging in place, and leaves many individuals with little choice but to stay in long-term facilities or lengthen their hospital stay.
Our non-medical transitional services are meant to offer the care and support you or a loved one may need to recover well and transition back to normal life from the comfort of home. Overall, our goals through this particular service include:
- Helping you to avoid prolonged hospital stays or admission to a long-term care facility
- Promoting a fast recovery by giving you one-on-one attention, tailored to your specific non-medical needs.
- Lowering your readmission chances by helping you to maintain your personal hygiene, reducing your risk of falling, and encouraging you to stick to your medication regime.
Some of the services included in our transitional care package include but are not limited to:
Before your release from hospital:
- Stocking up on food and other supplies you may need once you’re home.
- Cleaning your house in readiness for your return from hospital.
- Preparing a temporary bedroom well in advance so that you don’t have to go upstairs.
- Arranging things you may need, like food, dishes, clothes, TV remote, mobile phone chargers, and others at hip or shoulder level so you can reach them without straining yourself.
Arranging for special equipment you may need so you may have a safer and more comfortable place to recover from. Such arrangements may include:
- Having a shower seat installed
- Having your toilet seat elevated
- Having handrails and grab bars installed in your bathroom
Upon your release from hospital and during your recovery:
- We will transport you back home, taking much caution not to injure you or make your ride uncomfortable.
- We will receive non-medical care instructions from your doctors.
- We will communicate with your loved ones and provide them with important updates concerning your condition.
- We will do everything possible to ensure your safety and comfort.
- We will have a specially-trained caregiver by your side for as long as needed.
- We will clean up for you, cook nutritious meals, and assist you with mobility and toileting.
- We will provide you with a safe environment to reduce your chances of falling or encountering other home accidents.
- We will provide meaningful companionship and friendship.
- We will keep track of your scheduled medical appointments and transport/accompany you there.
Our specially-trained, non-medical, transitional care team works under the supervision and direction of an RN, MD or an experienced healthcare.
Our services are available 24 hours, 7 days a week, and we can arrange for daily visits, intermittent visits, overnight care, or live-in care.
